First Contact Teleplatform

– at night, on weekends and holidays, i.e. when primary health care facilities are closed, you will receive professional medical assistance by phone. To do this, call the Teleplatform at +48 800 137 200 or fill out the form. As part of the Teleplatform, you will receive the first contact:

• medical advice
• electronic prescription
• electronic referral to a doctor
• electronic sick leave
• referral for SARS-COV-2 test

The first contact teleplatform is available

• from Monday to Friday from 18:00 to 8:00 the next day.
• on Saturdays and Sundays and other days that are days off by law, from 8:00 to 8:00 the next day.

Currently, the hotline is operated in Polish, English and Ukrainian.

The form is available during the working hours of the First Contact Teleplatform.
